I am the parent of _________________________ and I am aware that gymnastics is a competive sport and rigerous training is involved when practice is attended.  I am also aware that gymnastics is a one year commitment, and if I withdraw from the program, I must let the coach know 2 weeks in advance.

You are aware that you will have to train 6 months to a year before your first competition.

This is only Competitive Gymanstics and not Recreational Gymnastics.


I agree to pay DSCGA $55.00 a month for my monthly tuition.  If I pay after the 10th, a 15.oo late fee will apply.
